" ... The most troubling aspect of the opioid crisis is the American death toll. According to the United Nations World Drug Report, the United States represents 4% of the global population, but over 12% of the world’s overdose deaths. One bright spot of the crisis response to date is that we have evidence of two key ingredients of reducing overdose deaths. The most immediately actionable steps are deploying Naloxone (Narcan) and Medication-Assisted Treatment (MAT) more broadly and more effectively so that they are readily available in the aftermath of opioid overdoses. Narcan, the overdose reversal drug, has saved countless lives when administered to people who have overdosed before they stop breathing. In The United States of Opioids, I highlight the way Narcan changed the role of first responders and police on the front line of the crisis. Meanwhile, MAT in its various forms (buprenorphine, naltrexone, and methadone) has emerged as the linchpin of evidence-based treatment for opioid addiction but remains inaccessible. According to the 2017 Presidential Opioid Commission Report, only ten percent (10%) of addiction treatment programs nationally offered any form of MAT. Between significant regulatory restrictions on prescribers and a legacy of MAT stigmatization, MAT is not nearly available enough.
